Form 1095-C Availability Notice
In accordance with updated IRS regulations, Otelco Telephone LLC is no longer required to automatically mail Form 1095-C to all employees. Form 1095-C provides information about the health insurance coverage offered to you during the 2025 calendar year. While this form is not required to be filed with your tax return, you should keep it for your records.
How to Request a Copy:You may request a free printed copy of your 2025 Form 1095-C by contacting the Benefits Department via:
